Biographical Narrative

Upon completion of medical school and residency training I entered a private practice of pediatric surgery. During my initial nine years in private practice I had some interaction with medical students and residents and found that I wished to increase my involvement with medical education and research. In 2003 I was recruited to Nationwide Children’s Hospital to both develop minimally invasive pediatric surgery as well as to lead the program in surgical critical care. Both of these opportunities allowed me to be much more involved with the teaching of medical students, residents, fellows and also with research. I have always been very patient focused and greatly enjoy the clinical work including both the surgical procedures and the pre and post- operative care. Now I am gratified to also have the opportunity to contribute both in education and research.

My main clinical focus has been in the development of new pediatric minimally invasive surgery procedures. I have introduced several minimally invasive procedures to Nationwide Children’s Hospital including treatment of abdominal testes (Fowler Stevens orchidopexy), Hirschprung’s disease, imperforate anus, diaphragmatic hernias, tracheoesophageal fistula/esophageal atresia and liver resection. I also established the Chest Wall Clinic for minimally invasive surgery of pectus excavatum and treatment of other chest wall disorders such as carinatum and chest wall tumors. Nationwide Childrens Hospital is now a regional referral center for the minimally invasive Nuss procedure for pectus excavatum as well as the traditional open surgical procedures. In addition, I have introduced external bracing as the main treatment for pectus carinatum, an effective non- operative option.

Education is an important component of my work. As Director of the Surgical Critical Care fellowship, I have mentored a fellow annually for the past nine years and all have passed their examinations with the American Board of Surgery for Added Qualifications in Surgical Critical Care. All have gone on to further training in pediatric surgery, and most are now practicing pediatric surgery and surgical critical care. Some of these fellows have worked with me for an additional research year doing clinical and surgical outcomes research. We have had several presentations at national meetings related to this work.

I also enjoy the educational time spent with medical students, residents and fellows at all levels of training. I have frequently mentored pre-medical students as well. In 2010 I was honored to be recognized with the Dr. Denis R. King Teacher of the Year Award for outstanding contributions to surgical education at Nationwide Children’s Hospital.

My scholarly efforts have been in clinical epidemiology and outcomes research to gain better understanding of disease processes in pediatric surgery and trauma. I have examined clinical case series but also have used large clinical registries and administrative databases to analyze the epidemiology and variation in care and outcomes relevant to pediatric surgery problems. My published investigations include participation in a large NIH sponsored multicenter randomized trial of surgical therapy for necrotizing enterocolitis, a study of a novel percutaneous therapy for lymphatic malformations, a multicenter registry evaluation
of intra-abdominal arterial trauma, analysis of advanced respiratory treatments for pediatric burn injuries and the use of anticoagulation to prevent venous thrombosis in pediatric trauma patients.

In the past two years I have formalized my interest in surgical quality improvement. I completed the Advanced Training Program in Healthcare Delivery Improvement through Intermountain Healthcare and was named the Surgical Director for Quality Improvement at Nationwide Children's Hospital. In this capacity I have worked on improved communications in the operating room and handoff communications from the operating room to the intensive care units. I have led projects to reduce surgical site infections for spinal, cardiac and neurologic shunt procedures. I am currently working on projects to reduce surgical site infections in all surgical specialties, enhance the care of patients with appendicitis, decrease the incidence of decubiti and skin injuries and reduce the use of perioperative blood transfusion. We have had success in several of these projects and some of these successes have been validated by acceptance for presentations of this work at national meetings. I continue to integrate quality improvement with research and teaching of the fellows and residents as I believe that improved patient care and education of health care professionals are synergistic goals.

National and international committee participation has been especially rewarding. I am the founding Chairman of the Task Force on Critical Care of the American Pediatric Surgical Association (APSA) having developed this group to address the educational needs of our members to improve the care of critically ill surgical patients. The two current projects of this committee are to publish a handbook of critical care and to develop an educational conference for the annual meeting. I am also a member of the APSA Surgical Quality and Safety committee. I work with the American College of Surgeons Pediatric National Surgical Quality Improvement Program (PedNSQIP). On the local level I am the Surgeon Champion for our hospital and coordinate data

collection and submission to PedNSQIP. At the national level I am a member of the PedNSQIP Measurement and Evaluation Committee. Finally, I have been a member of the Quality & Standards Committee of the International Pediatric Endosurgery Group combining both my interests in minimally invasive pediatric surgery and in quality improvement. In all of these efforts I have enjoyed working with surgeons across North America and from around the world.

Curriculum Development

Curriculum development is an important part of my work, both as a pediatric surgeon and as the Directer of the Surgical Critical Care Fellowship.

I am very active both as a participant and lecturer in the weekly rounds and tutorials for the medical students, residents and fellows on the pediatric surgery service . I also give lectures about pediatric surgery to the medical students during their surgery rotation. In addition to lectures, I engage in daily teaching in the outpatient clinic, hospital wards, and the operating room.

As the program director for the fellowship in surgical critical care, I assure that the fellows meet or exceed the requirements of the Accreditation Council for Graduate Medical Education (ACGME). I have made revisions of the curriculum including the initiation of daily bedside teaching rounds. These multidisciplinary rounds have been very well received by the trainees and by the other members of the health care team because they provide insight about patient care, improve communication and foster collaborative relationships. Most important, these rounds have enhanced patient care.

Another important revision is the coordination of educational efforts with Pediatric Critical Care and Anesthesiology. Based on feedback from faculty and fellows, several lectures and curricular activities have been reorganized to better fit the needs of the pediatric surgery critical care fellow. I also supervise the critical care fellow who presents several lectures per year to the Department of Pediatric Surgery on surgical critical care topics. The lectures are attended by the surgical faculty, general surgical residents, and medical students.

An additional initiative is the formal one month rotation in the Surgical Critical Care Unit at the Ohio State University Hospital which provides invaluable exposure to large variety of critically ill adult surgical patients.

The ACGME has reviewed our Surgical Critical Care program twice during my tenure and both visits have been successful and without any criteria deficiencies. We are currently accredited for 5 years, the maximum length allowed by the ACGME.

I have also been a member of the Department of Surgery Graduate Education Commitee.

Approach to and goals in teaching

I have had consistently favorable responses from students and residents for teaching culminating in my receiving the Dr. Denis R. King Teacher of the Year Award in 2010.

My approach to teaching is multi-dimensional and includes a combination of didactic lectures and case based discussions. The didactic lectures present to students and residents the principles of a particular pediatric surgery topic. Based around a single disease or clinical entity, we discuss diverse symptomatology that may be present in different patients. The case based presentations mainly occur at the bedside with each new patient at the time of clinical presentation providing a focused discussion about an individual patient. My goal is to help the students to understand the problems of the individual patient in the context of general pathophysiological principles. The case based discussions can be frustrating for students and residents who at times would prefer information presented to them in a more direct fashion. I try to use this sense of frustration and uncertainty to engage their imagination, to encourage them to organize their thoughts and to foster their ability to draw upon previously learned principles of surgery in an integrative fashion. This complex process is at the heart of good surgical pratice and is
difficult to teach. Medical students, who are often admitted for their ability to memorize information, are now being asked to apply that information to an individual case, a process that involves not only intellectual effort but the ability to communicate with a team of individuals caring for the patient as well as communicate with the patients themselves.

I also educate students and residents about the science of quality improvement as applied to patient care. Methodologies adopted from industry which can improve care delivery include the use of statistical process control, pdsa cycles, pareto charts and flow diagrams to name only a few. My new role as Surgical Director of Quality Improvement at Nationwide Children's Hospital affords me the opportunity to apply the tools to multiple surgical departments and the ability to add these practices to my teaching armamentarium.

Evaluation of Teaching

The means for evaluation of my teaching include reviews of each lecture and also surveys at the end of rotations administered by the continuing medical education office. I have paid close attention to these comments which have helped me to improve my instruction both with respect to content and style of teaching. One important element I have learned is related to bedside case presentations and discussions. The students and residents need to be reminded and encouraged that questions which arise in these discussions are not only intended to test their knowledge but more importantly to allow them the opportunity to integrate that knowledge with the clinical situation and to organize their response to a particular problem. As physicians they will be expected to apply that knowledge to an individual patient found in specific circumstances and be able to defend their analysis and plans for the patient. Unlike the didactic lectures, the question and answer format is challenging and intended to improve their confidence in making decisions.

I am also reviewed by my peers following didactic lectures and undergo an annual review by the department chief. These critiques have been very helpful in improving my teaching.

Focus of the Research

My research and scholarly efforts have been in two main areas. The first is in the development and application of minimally invasive surgery to infants and children. The second is in clinical epidemiology and outcomes research to gain better understanding of disease processes in pediatric surgery and trauma. I have examined clinical cases and case series of patients but also have used large clinical registries and administrative databases to examine the epidemiology and variation in care and outcomes relevant to pediatric surgery problems.

My major accomplishments are in the development of several new laparoscopic procedures for pediatric general surgery. These have included minimally invasive approaches for undescended testes, Hirschprung’s disease, diaphragmatic hernias, esophageal atresia and liver resection, in addition to further refinement of treatment for more common disorders such as appendicitis, gall bladder disease, gastroesophageal reflux and splenectomy. I also developed the Chest Wall Clinic for minimally invasive surgery of pectus excavatum and treatment of other chest wall disorders such as pectus carinatum and chest wall tumors.

My recent published investigations include: participation in a large, NIH sponsored, multicenter randomized trial of surgical therapy for necrotizing enterocolitis; a study of a novel percutaneous therapy for lymphatic malformations; analysis of a multicenter, trauma registry in evaluation of intra-abdominal arterial trauma; and a review of serious complication from central venous catheters. I published two book chapters including a review of the evaluation of cervical spine trauma and one concerning esophageal surgery.

I completed the certification for the Advanced Training Program of the Intermountain Health Care Institute for Health Care Delivery Improvement. This in-depth training in national health policy, consumer markets, and health care economics has given me better tools to improve health care delivery and patient safety.

My research interest will continue to expand on previous work in the development of minimally invasive surgical procedures. I continue to pursue clinical and outcomes research of pediatric surgery and trauma. I have also begun to be involved in quality improvement projects at Nationwide Children's Hospital to pursue better patient safety. I am hopeful that some of this work will have affects outside our hospital by creating policies which could become standard for other pediatric surgery patients.
